Health Requirements for Attendance

 

PreSchool
School Physical with state required vaccinations-due before 1st day of school and Lead Level

 

Kindergarten
School Physical with state required vaccinations and Lead Level -due before 1st day of school
Vision Exam by licensed Optometrist-due October 15
Dental Exam-due May 15

2nd Grade
Dental Exam-due May 15

6th Grade
School Physical with state required vaccinations-due before 1st day of school
Dental Exam-due May 15

9th Grade
School Physical with state required vaccinations-due before 1st day of school |
Dental Exam- due May 15

 

12th Grade
2 doses of meningococcal vaccine, unless the first dose was received at 16 years of age or older. -due before 1st day of school

Transfer Students
Transfer from an Illinois School-Health records will be requested from prior school; students enrolled in K, 6th or 9th grade-see above requirements
Transfer from an out-of-state school-School Physical completed on Illinois Certificate of Child Health Examination form with state required vaccinations; Vision Exam by licensed Optometrist
